I think people are getting the wrong end of this stick.
The story is that a "rising star" who wants to be mayor of New York has made himself look a prick. Literally.
I'm not saying the image is indecent. It's the message behind the sending of the image that's a bit pathetic. An older guy sending a 21-year-old that picture is saying volumes about how his mind works.
It would be totally different if he was knocking her off, but apparently not, I don't think they've even met. At least Clinton got a blowjob.
And the best bit of all is his contortions trying to wriggle out of it.
So I don't think it's about the american attitude to sex, it's about an older guy making a complete fool of himself trying to impress a younger chick, and in the process, showing the whole world how his sad mind works.
It's choice entertainment. Good old USA. Give us more!!

American television draws a distinction between broadcast free television, and pay television like cable and satellite television. The former are regulated by the FCC, because of the idea that the airwaves are public. Cable and satellite aren't, and they can pretty much show what they want, which is why on my cable television, they have all sorts of sex, violence, etc., and all the way up through porn. The Superbowl Nipple-gate incident was rather silly, but it was on the free television variety of broadcast. The same is true of radio. Howard Stern, for example, is completely uncensored on SiriusXm Radio. However, free broadcast radio has to refrain from getting too sexual or using too much foul language. There is no shortage of nudity, sex, violence and other such things on American television. I have 200+ channels on what amounts to basic digital cable, and I can go home and watch porn. Not on ABC, NBC or CBS, but on my cable system there are dozens of such options, including "on demand" options.
