What would a "straight mafia game look like", for instance in the case of Mafia IV?

The babies are becoming a touch less demanding now so I'm starting to work on setting up a Call of Cthulhu game. As with the paranoia game it will be more RPG based on mafia rules. Death and insanity await those brave investigators who dare to look into the unspeakable practices lurking beneath the surface of 1920s Arkham.
