The basic rate is fixed annually by the committee of the insurance companies, health inspectorate and patient reps as is the personal amount we have to pay for certain treatments. No insurance company can charge more than the basic rate but they can charge less (to attract more clients).
As an individual you can take on more insurance to increase cover but that is entirely your choice.

There is nothing really badly wrong with US care other than it is costly. And billing is too complicated (due to insurance). Many unnecessary procedures are done. Most of them in the last month of a person's life. Also, competition never came up as the system is a one size fits all system of care. A patient can only cut costs by making most care outpatient care. Ive had general anasthesia 4-5 times but have never been admitted to a hospital in 62 years. I've never been in an ER for me. My kids, 2 times.

That's like owning a Rolls Royce. There's no reason that everyone can't have one, except that they're costly.
Unfortunately, cost is the fundamental thing about almost everything. Without it, it would be a great world.

Same as here in the UK. You can pay in to private health insurance, without losing your state entitlement.
