An ISO image, which you burn onto a CD to boot from. Alternatively, you can boot it from a USB pen drive, if you use a utility like YUMI:

This might sound like being insanely complex, but it's actually very straightforward. If you run YUMI, you can tell it to add the Memtest ISO to the USB drive, and then choose that from the YUMI boot menu.

Regarding burning it to a CD, you need to specifically "Burn ISO image". If your burning software doesn't do that, then you'll need to download one that will. There's heaps of lite and free burning software on the net.
