You concentrate on whatever is bugging you, and then you think about it through the filter of the "answer", forces you to know your mind and what you think about the situation.
When you know the book intimately enough and use the yarrow stick method, you can even subconsciously control the answer and get something really relevant.

But sure, if you take it like a supernatural oracle, or use a purely random casting method, it's sheer bollocks.
