Mutations happen accidentally. Those mutations that have a beneficial effect result in evolution.
It is a mixture. Some, if not most, evolutionary changes occur because of natural selection on a variable (via mutation) gene pool. Other changes are random, in the sense that they are neutral in terms of survival.

The Vatican announced that Francis had changed the catechism of the church — the compilation of official Catholic teaching.

Previously, the catechism said the church didn't exclude recourse to capital punishment "if this is the only possible way of effectively defending human lives against the unjust aggressor".

The new teaching says the previous policy is outdated and that there are other ways to protect society.

It has often struck me how fortuitous it is that DNA does make small mistakes in copying itself now and again. Without that little glitch there would be no evolution - at least of the kind we know. That would I assume mean that single cell life would never have evolved.
