No, because that then changes Dr Brian’s liability insurance status. He’s no longer simply an employee performing the surgery, he has now paid for it, he’s a partial consumer/customer, which will invalidate his liability insurance.

ACA only put premiums under control. The rest is a mess.

Other healthcare giants also rank in the top companies in the US and world: CVS, a pharmacy that now also owns the health insurance company Aetna and pharmacy benefit manager Caremark, is the sixth on the US list.

I'm sympathetic about burnout. They're just swamped all the time. I can see this in their messaging too, with campaigns desperately trying to educate people about when to visit a doctor vs the emergency room, and when you should just stay home. This study claims some 10% of ER visits are non-urgent. It has to be possible to reduce that.
