The source where I found a version of the image had cropped it. The full image includes some figures hovering in the background above Trump. It seems that between the original posted by some MAGA stooge and Trump posting it, somebody altered the central figure to include three horns, which may be a reference to biblical prophesies about the supposed world-ending apocalypse.
