Will restaurants that don't allow dogs be guilty of a hate crime?
Or girls who don't fancy ugly men?
Or leisure parks rides that have a minimum height rule?
Or Muirfield golf club and their no women rule?

The world is so full of hate.
While there is a market for shit, there will be assholes to supply it.

Hating dog is not a crime. Nor hating dog owners. Victimising dog owners is a crime, and victimising dogs may fall foul of the UK's rather scant animal cruelty regulations. There are legitimate food hygiene and safety reasons for excluding dogs in restaurants - with an exception for guide dogs, as that excludes the visually impaired from accessing services. And besides, everyone knows that a guide dog always has clean paws.

As the law stands at the moment, there is no statute or local ordinance which obliges girls, or young or old women, to find ugly men attractive - however, you never know your look.

Again, this is a health and safety matter which attempts to mitigate the possibility of harm. Sometimes there are similar restriction based on height, size or weight. One can hardly consider this an arbitrary or discriminatory exclusion.

On the other hand, this is an arbitrary and discriminatory exclusion of a portion of the population based on the possession, or lack thereof, of certain and particular physical attributes. However, a private club may order their affairs in accordance with the law in any way they see fit - so ideally the club's admission and membership policy has to be tested in law.
Personally I wouldn't wand to be the member of a club that excluded women or blind people.

Actually, no, there aren't. There's no contemporary cases of anyone being made ill by a dog in a restaurant. It's a long-held false belief and bigotry and nothing more that dogs are "unsanitary" and pose a public health risk in restaurants. There's not a shred of evidence that this is true. France allows dogs in restaurants and we don't see French people dropping dead of dog-borne diseases. The biggest health risk in any restaurant isn't a dog, it's the wogs who are cooking your food who may have hepatitis and fail to wash their hands after taking a shit and the owner who doesn't keep the place clean and the food stored properly.

The only real problem with dogs anywhere is the potential for dog misbehavior, but that's easily rectified if and when it happens...you simply kick the owner and the offending dog out...and you can do that even if it's a service animal if it acts up or is disruptive...under the ADA.
